UPDATED:
These athlete and coaches members have not sent in their
waivers that print with their USA Cards when purchased online. You must send these in to me immediately to
ensure that you are covered. If you know
someone on this list or are a coach of someone on this list or are a club who
is holding on to these waivers, please have them sent to me (not USA Wrestling
as I previously stated at the fall meeting).
Clubs will also want to keep a copy on file for 4 years as required.
If you purchased your card before
This list will be updated once a week if possible.
If you lost the waiver you can log back into your account
and re-print the card and waiver:
You will need to go the the following page:
https://webpoint.themat.com/wp15/IntraLock/Login.asp
And type in the user name and password exactly as you had it when you
created the account.
Make sure you follow lower and upper case letters or it will lock
you out.
When you get logged in, click on My acccount on the left side of the
page and when the next screen comes up, click on the tab called Membership Info
and it will bring up the card purchase history.
Then you will then click on a little square that looks like a white piece
of paper:
When that comes up, it is the confirmation from the purchase. You can
print that out and your USA Card and waiver will be on the forms.
You will then logout of that account.
Send the waivers to me ASAP to clear your account:
